A plain-language overview of website form fields, email drafts, maps, and optional location sharing.
Contact Form
Required: full name, email address, and message. Optional: phone number. Purpose: prepare a message requesting contact from the practice. The form creates an email draft; it does not submit information into a website patient-record system.
Callback Form
Required: full name, phone number, and preferred location. Purpose: prepare an appointment-request email. Sending the draft does not reserve an appointment or guarantee a callback time.
Where Form Information Goes
The form values are held in the current page while you fill it in and placed in your email draft on submission. No website account, patient portal, or form database is implemented for these requests. Once you send the email, your email provider and the practice’s mailbox handle it. Ask the practice about retention and deletion of received enquiries; no retention schedule is asserted here.
Maps and Location Permission
An enabled office map loads content from Google. Use My Location asks your browser for permission and, if granted, uses your coordinates in Google Maps directions. This permission is optional and is not needed to call or request an appointment. Manage location permission in your browser settings.
Technical Data and Cookies
Hosting and third-party services may process network and browser information. Google Maps may use cookies or similar technology under its own policies. The website’s current contact and callback features do not use a local-storage account or patient profile.
No advertising tracker or analytics script was identified in the website code reviewed for this page. Hosting-level or externally configured tracking has not been verified. Cookie and tracking disclosures must be reassessed before adding analytics, advertising tags, or other integrations.
Control and Questions
