Privacy Policy
How United Dental Group’s website handles appointment enquiries, personal information, and dental privacy concerns.
Scope of This Website Policy
This policy describes the website’s contact and appointment-request features. It is not the practice’s HIPAA Notice of Privacy Practices and does not describe all handling of dental records, insurance information, or clinical care. Ask the practice for its current patient privacy notice before providing clinical information.
Information You Choose to Provide
The contact form asks for your name, email address, optional phone number, and message. The callback form asks for your name, phone number, and preferred location. These details are used to prepare an enquiry or appointment request, not to diagnose or treat a condition.
Email Requests and Sensitive Information
Submitting a form opens a draft in your email application addressed to the practice. The website does not automatically send the message or confirm delivery. You must send it yourself. Your device, email provider, and the recipient’s mailbox handle the message under their own security and privacy arrangements.
Do not include diagnoses, medical history, insurance identifiers, payment details, X-rays, or other sensitive health information in these forms. Ordinary email is not a secure patient portal. Call the practice to ask how to share clinical information safely.
External Services
Website hosting may process connection and request information such as IP address and browser details. Google Fonts and an enabled Google Maps embed may receive technical information when loaded. External links and your email application are governed by those providers’ policies.
Location sharing is optional and requested only when you choose Use My Location. If permission is granted, coordinates are used to display directions through Google Maps. You can decline and use the location’s written address instead.
Your Requests and Patient Rights
For questions about an enquiry, correction of contact information, or the practice’s handling of patient information, call your preferred location. Health-record access, amendment, restrictions, complaints, and other applicable patient rights are addressed in the practice’s patient privacy notice. This website cannot verify identity or fulfill clinical-record requests.
Children and Caregivers
A parent or guardian should contact the practice on behalf of a child. Do not submit a child’s health details through the website forms.
Practice Review
This website-specific policy is a draft for practice review. The practice must confirm its privacy contact, vendor arrangements, retention practices, and applicable legal disclosures before adopting it as its official policy.
